Is this Barack Obama's birth certificate or a fake?

This "birth certificate" is being circulated by Obama supporters as the authentic birth certificate of Barack Hussein Obama, II. Is it?  I don't know.  As someone who has done quite a bit of investigative research, the birth certificate (see below) seems strangely lacking information at least some birth certificates include.  Why?  Here is a list of reasons I question the authenticity of this document:

1. Certificate number is not displayed.

2. No place of birth is listed (e.g., specific hospital).

3. No street address for the mother or father.

4. No age of parents listed.

5. No informant's signature.

6. No doctor's signature.

7. No attendant at birth listed.

8. No mention of any serologic tests.

9. No seal or stamp of the department responsible for live birth certification.

10. It appears to have a "revision" date of November, 2001.

Barack Obama needs to give the Hawaiian Department of Health permission to release his birth certificate which the governor of Hawaii off-limits to investigative reporters and document experts.  This will be the only way to once-and-for-all establish the truth or Barack Obama's citizenship and right to run for the Presidency of the United States according to the U.S. Constitution.
